How To Ask Work From Home to Manager?

When asking to WFH from your manager, the WFH request form/email should include important information about the employee, the type of WFH requested, how long WFH will last, and a short description. Check out the following steps to write a WFH request form:

  1. Information about the employee – The form should include all relevant data such as the employee's name, title, department, and contact data. Sometimes, employees working from home don't have access to the company's email; thus, make sure you have the correct contact data.
  2. The description of the request – A WFH request form should include a short description of the request. In the description, the employee should mention details about the request, such as the reason she/he requests WFH time (e.g., illness, impossibility to get to work, personal and family issues, COVID quarantine, etc.). Moreover, the WFH conditions (e.g., requiring technical equipment, non-existing Internet connection, etc.) are needed, and how much of her/his tasks are manageable from home.
  3. The type of WFH time – In some situations, WFH has requested a rare event, and the employee needs only one day of WFH. In other cases, the employee is forced to work from home for a more extended period or even for an undetermined period. Make sure your WFH request covers all these situations and provides dedicated space for mentioning all the details.
  4. Signatures – A work from home request form must include the employee's signature and the request's date. It also must have the signature of the person who approves the request and the approval date.

Subject line: Request to work from home due to childcare

Dear Sir/Madam,

I am writing this email as the follow-up of the dialogue we had on [Add date here], regarding my request to work from home during the current pandemic conditions.

These days, my children will not attend school as a result of the pandemic, so I am required to watch over them.

My request is to work from home until the current pandemic situation is stabilized. I believe that my role and tasks allow me to do this effectively without diminishing my productivity levels.

I am not feeling that well enough to be able to go to the office. I have been to a doctor this morning and I am on medication now for cough, headache, and mild fever.

Thank you for considering this request to work from home. I would appreciate it if you can confirm in an email my request, so I can finalize my arrangements.

Yours faithfully,

[Your name]

I would appreciate your approval for my request to work from home.

Yours faithfully,
